Because we provide behavioral health, counseling, mental health, and substance-use-disorder treatment services, information submitted through our websites may include sensitive health, mental health, or substance-use information.
Please do not submit detailed medical, mental-health, substance-use, or other highly sensitive information through a general website contact form unless specifically requested by the Practice.

Mental Health and Psychotherapy Information
Mental-health information is protected health information when subject to HIPAA.
Certain psychotherapy notes receive additional protections under HIPAA. Psychotherapy notes are maintained separately from the patient’s general medical record when applicable and are subject to additional privacy protections.
Website contact forms are not intended for submitting psychotherapy notes or detailed clinical information.
Substance-Use-Disorder Information
The Practice may provide treatment for substance-use disorders. Certain substance-use-disorder patient records may be subject to additional federal confidentiality protections under 42 CFR Part 2, in addition to HIPAA.
The Practice maintains policies and procedures designed to protect information subject to these requirements.
Where applicable, the Practice’s Notice of Privacy Practices and Part 2 Patient Notice explain additional protections, permitted uses and disclosures, and patient rights concerning substance-use-disorder records.
Communications and Consent
Providing your telephone number or email address allows the Practice to respond to an inquiry or request.
Marketing and other non-essential communications require separate consent where applicable. Consent for calls, emails, and text messages is obtained separately and is never a condition of receiving a response to your inquiry.
Emergency Communications
Our websites are not intended for emergency or crisis communication.
Do not use a website contact form to report an emergency, suicidal crisis, or other urgent mental-health or medical concern. If you are experiencing an emergency, call 911 or seek immediate emergency assistance.
Privacy Rights
Patients may have rights concerning their protected health information, including rights to:

- Request confidential communications.

Our Notice of Privacy Practices provides a more detailed explanation of how protected health information may be used and disclosed, our responsibilities, and patient privacy rights.
For services involving substance-use-disorder treatment, the applicable notice also addresses the additional confidentiality requirements governing Part 2 records.
